This questionnaire addressed subject ear history and medical conditions that may be associated with the onset of auditory dysfunction; occupational history (e.g., previous jobs exposed to noise, use of solvents, use of hearing and respiratory protection at work, and tenure at each workplace); and non-occupational noise exposure.
