Without ECMO, difficulty maintaining adequate alveolar ventilation is one limitation to the use of a protective ventilation strategy – exposing patients to potential side effects of subsequent hypercapnia, such as intracranial pressure elevation, pulmonary hypertension, depressed myocardial contractility and a reduction in renal blood flow [ 56, 57].
