A number of programmes have already begun reassigning tasks as a result of shortages in specific classes of personnel, including assigning nurses to evaluate patients for ART and prescribe in uncomplicated cases, and shifting counselling and education from nurses to lay counsellors and trained peers (Table 1, Table 2).
Consequently, there is a need for 'something else' for the triggering of more pathogenic events and an amplification of iRBC-triggered changes by host immune responses is implicated in the shift between uncomplicated and severe malaria (Grau & de Kossodo, 1994) (Fig. 2).

This suggests that a shift from a "severe" to an "uncomplicated" transcriptional profile may occur within a very short time, and could explain the "uncomplicated" and benign gene immune transcriptional responses reported by Long et al [28].
Conclusions This study identified factors associated with normal pregnancy through adoption of a novel hypothesis generating approach, which has shifted the emphasis away from adverse outcomes towards uncomplicated pregnancies.
A shift toward a higher susceptibility of children older than five years was also noted for uncomplicated malaria, suggesting an epidemiological transition of the infection in the country.
However, while there is a general acceptance that the same shift in peak age occurs for severe malaria [3] [8], there has been limited evidence of this for uncomplicated clinical malaria [9], [10].
