In Fig. 9 (left), we show data on the rate of documents becoming unfit, i.e. developing at least one large missing piece per 100 sheets, as they are handled (leafing through the entire document once as if reading/searching for information).
Overall, the rate of documenting and monitoring of patients' use of DS by U.S. pharmacists was low.
