The implicated populace was a community of Injection Drug Users (IDU), who documented an HIV prevalence approaching an outstanding 27%.

The presence of quinolones in 50% of the most common MDR profiles and the high resistance among the main bacteria implicated in community-acquired UTI may be explained by the high consumption levels.
The aim of this study was to assess the most frequent multidrug resistant (MDR) profiles of the main bacteria implicated in community-acquired urinary tract infections (UTI).
Unfortunately there are few publications about the main uropathogens implicated in community-acquired UTI and their antimicrobial resistance pattern, when compared with UTI acquired at hospital level.
For these cases, the carbapenem imipenem, the third and fourth generation chephalosporins, aminoglycosides and glycopeptides (for Gram negative bacteria) should be suitable as the bacteria more implicated in community-acquired UTI do not present high resistance to these drugs.
Although E. coli was the most frequent uropathogen implicated in community-acquired UTI (being implicated in more than an half of all the UTI), as frequently detected in other studies [ 10, 13, 16, 17], significant differences in relation to the other bacteria involved in the community-acquired UTI were observed relatively to other studies performed at the community level.
E. coli is known to be one of the major organisms causing nosocomial infections within the hospital and has also been implicated in community acquired ESBL [ 19, 20].
These programs go far beyond small clinical research projects and directly implicate communities and whole populations.
According to the literature, quinolone resistance is associated with ESBL-production, being the production of CTX-M, enzymes common among bacteria implicated in community-acquired UTI, namely, Enterobacteriaceae such as E. coli, a serious cause of community-acquired UTI worldwide [ 28].
Of the 204 ESBL producers, 66 (32.4%) were implicated in community-onset infections.
