The inclusion criteria were: (1) history of acute or chronic (more than three episodes of dislocation during a period of more than 2 months) anterior instability of the shoulder, (2) strong clinical suspicion of shoulder instability, (3) MR arthrography of the affected shoulder and (4) subsequent arthroscopy.
In case of suspicion of joint instability, weight-bearing CBCT provides information about joint alignment [5, 8] although this can only be performed on dedicated CBCT equipment [20].
In 12 patients the clinical suspicion of chronic instability of the syndesmosis was confirmed during arthroscopy of the ankle.
In patients with chronic complaints after an injury, the symptoms and signs as mentioned above in combination with the mechanism of trauma, and a thorough physical examination, which includes tests to assess the integrity of the ligaments of the syndesmosis, should arouse a strong suspicion of chronic instability anterior syndesmosis [ 19, 20].
We found that as the complexity of the fracture increases, the degree of radial shortening also increases; thus, the index of suspicion for instability should increase.
Indications for surgery included haemodynamic instability; suspicion of, or risk factors for, rupture; contraindications to methotrexate or failed medical treatment.
Patients with acute rejection or clinical suspicion of rejection or clinical instabilities and patients who were under hemodialysis treatment at the time of approach or had recent surgery or fractures, coagulopathies, thrombotic diseases, or acute infections or were suspected of infections on the day of blood collection were excluded from the study.
The study population consisted of 40 consecutive patients (18 women, mean age 68 years, range 40 81 years and 22 men, mean age 68 years, range 46 87 years) referred for radiography and digital linear tomosynthesis with suspicion of prosthesis loosening with increasing pain or instability or for preoperative assessment.
