WASHINGTON — Inadequate training and supervision of key personnel aboard the Deepwater Horizon drilling rig and a lack of focus on safety were among the factors that contributed to the explosion that killed 11 workers and sent millions of barrels of oil gushing into the Gulf of Mexico, according to a preliminary report released Wednesday by a team of scientific experts.
