To assess the influence of excessive daytime sleepiness on clinical features and impact of migraine.
Positive airway pressure (PAP) therapy is the most commonly used medical modality to reverse the apneas, hypopneas and inspiratory flow-limited breaths which result in the oxyhemoglobin desaturation, altered sleep architecture, and daytime sleepiness representing the cardinal features of obstructive sleep apnea/hypopnea (OSA/H).
Both types of narcolepsies belong to the group of central hypersomnias (CH), a spectrum of poorly defined diseases with excessive daytime sleepiness as a core feature.
First, excessive daytime sleepiness (EDS), the key feature of narcolepsy, is shared by many other central hypersomnias and also a prevalent life-style consequence of insufficient sleep in our modern societies.
Taken together, our data provide evidence that daytime sleepiness is a distinct feature of DLB that is not contingent on disease stage or on any one of the four core features of DLB.
These data suggest that abnormal daytime sleepiness is a unique feature of DLB that does not depend on nighttime sleep fragmentation or the presence of the four cardinal DLB features.
If daytime sleepiness is a unique clinical feature of DLB, this has implications for improved early detection and differential diagnosis of DLB, for consideration of alternate treatment interventions, and for promoting our understanding of the pathologic and neuroanatomic involvement in DLB.
It is characterized by recurrent airflow obstruction with symptoms and features such as excessive daytime sleepiness or hypersomnia, snoring, morning dry mouth upon awakening, and chest retraction during sleep [ 2].
As such, neuronal loss and Lewy pathology in brainstem and limbic regions, without widespread cortical involvement, are sufficient to produce daytime sleepiness, dementia and the other core DLB features.
However, the prevalence, clinical features, and impact of excessive daytime sleepiness among migraneurs in population-based setting have only rarely been reported.
These data provide evidence that daytime sleepiness can be distinguished from the other core DLB features, including fluctuations.
