Chief Justice Sears speaks frequently about marriage, saying that divorce and custody cases make up an increasingly large share of caseloads and that children suffer when they are raised out of wedlock.
In a sample of 20 of America's largest counties, the black share of the caseload rose by less than 1% between 1996 and 1999; the proportion of Hispanics rose by 2.7%, and that of whites fell by 3.3%.The report points out that those left behind on welfare are likely to be trapped by concentrated urban poverty.
Key features of ACT are: assertive engagement, a small caseload (maximum of 10 patients per clinician); shared caseload (i.e. all clinicians collaborated closely on each patient using one treatment plan); and community-based and assertive services on a time-unlimited basis [ 5, 19].
ACTE scored high on the following components of ACT: the small and shared caseload, and time-unlimited services [ 31, 32].
Table 1 shows that the main differences between ACTE and CAU will concern team approach including a shared caseload, contact frequency, and the location of care provision.
Better engagement was associated with a smaller caseload in ACT than in TAU, and with a shared caseload [ 10].
Critical components of ACT associated with reducing hospital admissions were shared caseload, community based services, 7x 24 hour services, a team leader who participated in patient care, full responsibility for treatment services, daily team meetings and time unlimited services [ 6].
These findings may speak to patterns of consultation or shared care at lower caseloads versus a specialist acting as the 'primary care provider' , measured as the provision of high proportions of all care, at higher HIV caseloads.
These urban counties, which contain about a third of the country's population, saw their share of the national welfare caseload rise from 48% in 1994 to 58% in 1999 (see chart).
