Serious respiratory insufficiency was recorded in one patient, who received artificial respiratory ventilation for 6 days, with catecholamine support of cardiovascular circulation.
While pneumonia caused by methicillin-susceptible Staphylococcus aureus (S. aureus) should be treated with oxacillin or flucloxacillin, methicillin-resistant S. aureus should preferably be treated with vancomycin, if no serious renal insufficiency is present (B-II).

The remaining 182 subjects were randomly assigned to treatment following the metformin run-in period (Fig. 1); one patient in the IDegAsp group was excluded before receiving insulin treatment due to a serious adverse event (respiratory insufficiency).
