Dual-energy X-ray absorptiometry (DXA) scans were performed at the 5- to 7-year follow-up with a GE Lunar Prodigy Advance 2005 DXA scanner; analyses were performed using enCORE software version 11.40.

All data were acquired on a Siemens 3-T Tim Trio scanner, and all analyses were performed in SPM5 (Wellcome Trust Centre for Neuroimaging, London, UK).
