We have previously shown that whole-cell vaccination using a K562/GVAX product can lead to clinical responses in patients with residual CML after suboptimal responses to TKI therapy.
Downregulation of Dicer was significantly related to tumor stage progression and downregulation of Drosha was significantly related to a suboptimal residual tumor size >1 cm after cytoreductive surgery.
The first published work (Berchuck et al, 2004) performed in 2004 used Affymetrix U133A GeneChip microarray to explore expression patterns in association with optimal (⩽1 cm residual disease) vs suboptimal debulking surgery (>1 cm residual disease).
This group also had better surgical outcome, defined as optimal (<1.0 cm residual tumor) versus suboptimal cytoreduction, and overall survival that was consistent with a stronger tumor immune response seen in those patients.
In univariate analysis of OS, platinum resistance, more advanced stage of disease, and suboptimal residual tumour at primary surgery were also found to be associated with a high risk of death of disease (Table 2).
