It is correct that we and others have reported that a high level of total cfDNA implies a poor prognosis in itself (Spindler et al, 2012, 2013a, 2013b, 2013c, 2013d; Hansen et al, 2014).
We found that BMI was an independent predictor of lower mortality with an HR of 0.98 per kg/m2 (95% CI 0.97 to 0.99, P = 0.041), suggesting that although diabetics had a poorer prognosis, diabetes itself had a weaker association with mortality than BMI (Table 2).
