The putative neuroprotective effects of antiplatelet compounds and the angiotensin II receptor antagonist telmisartan were investigated in the Prevention Regimen for Effectively Avoiding Second Strokes (PRoFESS) trial.
With regard to antiplatelet agents, the perfect example is redesigning of the ongoing PRoFESS trial by eliminating aspirin from clopidogrel arm after the earlier MATCH trial results became available.
This finding is consistent with a recent meta-analysis and the PRoFESS trial showing that when compared to clopidogrel monotherapy, long-term aspirin-based dual-antiplatelet therapy is linked to higher risk of intracranial haemorrhage among patients with ischaemic stroke.
These results are supported by the combined analysis of the Prevention Regimen For Effectively avoiding Second Strokes (PRoFESS) trial and the Telmisartan Randomised AssessmeNt Study in ACE iNtolerant subjects with cardiovascular Disease (TRANSCEND) trial, in which telmisartan reduced the risk of the composite outcome of cardiovascular death, myocardial infarction, or stroke compared with placebo.
The ProFESS trial also compared the efficacy of prophylactic treatment using aspirin 25 mg daily plus extendedrelease dipyridamole 200 mg twice daily (n = 10,181) versus clopidogrel 75 mg daily (n = 10,151) and either telmisartan 80 mg daily (n = 10,146) or placebo (n = 10,186) on reduction of disability and recurrent strokes 97 in patients with ischemic stroke.
