As no information on reasons for seeking ambulatory care is routinely collected in Switzerland, we derived clinical conditions for physical therapy referral from three sources: inpatient diagnoses and procedures, dispensed outpatient drugs and the specialization of the referring physician.
The hd-PS algorithm prioritizes variables within each data dimension (e.g., inpatient diagnoses, inpatient procedures, outpatient diagnoses, outpatient procedures, dispensed prescription drugs) by their potential for confounding control based on their prevalence and on bivariate associations with the treatment and with the study outcome [ 16, 19].
However, targeting the materials would have required the pharmacy staff to alter their routine dispensing procedures, and this effect is difficult to achieve.
The second routine, dispense radioactivity, aliquoted the requested radioactivity using the calibration file created previously in the start calibration procedure and operator-specified parameters such as the requested quantity in units of volume or radioactivity.
The procedure of dispensing the topical anesthetic gel was non-invasive and entirely painless as the gel was applied directly to the soft tissue of the periodontal pocket, and it was allowed to saturate into the gingival crevice.

Informed consent (parent and children) was received before the administration of any study procedure or dispensing of study medication in accordance with the ethical standards of the investigative site's institutional review board and with the Helsinki declaration of 1975, as revised in 2000 [ 15].
To date, it was shown that the share of generic drugs differed by prescription and dispensing procedure (ie, prescribing and dispensing separated or integrated) and larger hospitals used less generic drugs [ 25].
Mr. Langewiesche points out that ordinary bureaucratic procedures were dispensed with at the World Trade Center site and that the breakdown of traditional hierarchies made for a chaotic but energizing atmosphere, "a new social contract" in which "all that counted about anyone was what that person could provide".
