Demographics, injury type and severity, fistula characteristics and daily output, complications, interventions and cost were recorded.
All project informants were assured that their projects would be de-identified in our reports because of anticipated sensitivities about publication output, failed interventions or lack of real world impact.
Of particular interest are policies broadly labelled output market interventions, including those that influence the price of food production and those that affect trade liberalisation and public-food distribution systems (table 1).

Cost consequences were then calculated from the UKPDS outputs for the intervention and comparator models, and worked out as a cost per 1% reduction in absolute risk.
