Gov. Eliot Spitzer had ordered the report detailing what went wrong and how it could be avoided in the future.
Senator John McCain, the Arizona Republican who is the chairman of the Commerce Committee, ordered the report from the General Accounting Office of Congress late last spring, when high gasoline prices were prompting outrage among motorists.
General Barno ordered the report in May, and it was completed in June, but the release of any of it has been repeatedly delayed.
David Cameron ordered the report to be carried out by an MoD civil servant not involved in the programme and a civil servant from another department.
"Some of the figures, like the suicide rate for young American Indians, are just heartbreaking," said Dr. Thomas R. Frieden, the C.D.C. director, who ordered the report compiled.
Representative Frank R. Wolf, Republican of Virginia, who ordered the report for the House Appropriations Subcommittee on Transportation, said the timetable confirmed concerns raised in Congress about the tunnel dangers and the slow progress Amtrak had made in correcting them.
"A big earthquake is overdue in the region, and we're not naïve to that reality," said Mayor Gavin Newsom, who ordered the report in July and is working on legislation to make earthquake safety upgrades mandatory on soft-story buildings.
Mr Drakeford had originally ordered the report be carried out back in October.
The Association of Chief Police Officers in England and Wales ordered the report from the anti-corruption group Transparency International.
When he ordered the report, Mr Davey said the sector faced "unprecedented challenges that require new thinking".
