"At the beginning, four years ago, we were 22-year-olds going into 50-year-old clinicians' offices saying: 'Let me tell you about a solution to make your physiotherapy better.' Some were sceptical but over time that has changed".
Participants felt that other clinicians, especially older clinicians, think antibiotics are harmless.
They were concerned that changing the practice of providers would be difficult, especially for older clinicians.
First, it clearly shows the clinical potential of whole-exome sequencing, an approach that older clinicians might have dismissed, even if available, as a fishing expedition.
For example, Kuhn and colleagues (2014) found that although all VA providers expressed fairly high interest in integrating a smartphone app into posttraumatic stress disorder treatment, younger clinicians and those with smartphones found the app more usable than older clinicians and those without smartphones.
In contrast to some earlier studies which found younger clinicians to be more willing to intervene than older clinicians [ 39, 41, 42], this study found no impact of age or gender on confidence in diagnoses of problem drinking or advice given to reduce drinking.
This may be attributed to the deep-rooted attitudes of older clinicians, who are resistant to change their practice in accordance with present-day guidelines and who are highly confident in their knowledge based on their long years of medical experience.
Students suggested that the incorporation of technology may be a primary barrier for such older clinicians wishing to keep abreast with the current evidence but not possessing the necessary computer skills to do so (e.g. searching Medline and retrieving articles efficiently).
Participants who met the following inclusion criteria were invited to participate in the study: adults 18 and older; clinician-confirmed diagnosis of moderate to severe plaque psoriasis; confirmation by clinician that patient is being treated for psoriasis with either conventional (including PUVA) or biologic systemic treatment; and willing to be audio-recorded during the interview session.
"There is a sensibility among some old-school clinicians that they have a better sense of their patients' experience than patients do themselves," Dr. Basch said.
The institute will develop new courses on the patient-doctor relationship and rely heavily on junior faculty members and older practicing clinicians as mentors.
