Canada provides an interactive map service for chronic diseases, which can be customized with several parameters (disease, age, notification year, sex and data type) [ 21].
There is often inadequate oversight of glycemic control related to infrequent review of glycemic trends, complex and difficult-to-read glucose logs, and lack of specific diabetes treatment algorithms including glycemic parameters for provider notification (127).
On receiving the notification, MAG 2 is able to determine the MN's identity (ID) based on the parameters in the received notification signal.
We used data from World Health Organization (WHO) notifications and other published literature to inform parameters in our model (Table 1) [ 17, 18].
