To the Editor: Tschudin-Sutter et al. provide convincing evidence of transfer of an extended-spectrum β-lactamase producing Escherichia coli strain from a mother to her vaginally delivered twins, then from the neonates to a health care worker and other neonates in a neonatal care unit (1 ).
The birth registry at KCMC is a daily activity including public holidays with integrated neonatal registry for neonates admitted to a neonatal care unit.
In LUNESP, about 11% of neonates cared for by the intervention TBAs were targeted for AFR, a rate that is consistent with the incidence of serious bacterial infections during the first 28 days of life.

Healthcare-associated infections (HAIs) are frequent and critical complications associated with hospitalization of neonates, especially very low birth weight (VLBW) neonates, in neonatal intensive care units (NICUs) [ 1, 2].
Although the birth attendants were expected to maintain regular contact with the mother and infant pair during the first week after birth and to refer neonates for care if they appeared unwell, intervention birth attendants were also trained to recognise cardinal symptoms and signs of possible sepsis (see web extra on bmj.com).
