The first mutation that starts the first abnormal cell down its path to malignancy can be caused by any number of factors: genetic predisposition, radiation, chemical agents, viruses.

Some of these tiny uORFs could be artefactual as a result of point mutations that insert an in-frame start and/or stop codon in the 5'-UTR.
