It is of great clinical importance to be able to provide reliable estimates of cancer risk among individuals of those mutation-negative families for which mutation status will provide no further information; at our institute, almost three-quarters of the analysed families (unpublished observation).
Inappropriate responses (e.g., participants who responded that they knew their mutation status but provided responses in Section B) were not included in the statistical comparisons.
We analyzed large pathology datasets accrued by the Consortium of Investigators of Modifiers of BRCA1/2 (CIMBA) and the Breast Cancer Association Consortium (BCAC) to reassess histopathological predictors of BRCA1 and BRCA2 mutation status, and provide robust likelihood ratio (LR) estimates for statistical modeling.
Lakhani and colleagues [ 54] showed that information on age at diagnosis, high histological grade and an ER negative receptor status provides a powerful predictor of BRCA1 mutation status.
A nonprofit status provides far less flexibility.
Next, we asked if we could identify modules associated with TS mutation status, which would provide insight into dysregulation of molecular networks in TS and disease pathophysiology.
Primary GBM, which constitute 90-95% of GBM [ 9], differ from non-GBM with respect to DNA copy number [ 11], gene expression [ 11], DNA methylation [ 12- 14], and isocitrate dehydrogenase (IDH) 1 or 2 gene mutation status [ 11- 16], providing a molecular rationale for the two broad glioma subtypes of GBM and non-GBM.
This finding supports the hypothesis that the presence of 13-gene signature already reflects the p53 wild-type status, but also the p53 pathway functional activity and then the inclusion of p53 mutation status does not provide significant new discriminatory information.
However, the use of established and emerging tissue biomarkers will aid in the interpretation of difficult melanocytic lesions, provide prognostic information, identify mutation status, and allow the selection of patients who may benefit from targeted therapy.
