Distances are from the municipality centre to Ringerike Sykehus (RS) and Hallingdal Sjukestugu (HSS).
Using the 1995 census as the sample frame for the seven geographical regions (West, Pacific-south, Segovias, North, Centre, Managua and Caribbean coast), we stratified by type of settlement (regional capital, departmental centre, municipality centre, rural settlement) proportional to population.

The transition to municipality-based Centres for Youth and Family implies a change in settings, but in line with the indicators of a paradigm shift, such a shift would also imply a change in instruments with a growth in targeted and preventive interventions.
With it came a strong emphasis on both preventive interventions and the role of the family (Ministry of Youth and Family 2007).2 The Ministry developed municipality-based Centres for Youth and Family as easy-access organizations where parents could turn to for help and advice.
Previously, they were classified into 10 "urban" (lying completely or partially within borders of the city proper) and 7 "suburban" municipalities, whose centres are smaller towns.
