If the local staging has demonstrated the primary tumour to be locally advanced (T3/T4), have associated extramural venous invasion or multiple involved local lymph nodes, then the probability of liver metastasis is increased and further imaging should be actively pursued for lesion characterisation.
The most common involved lymph nodes sites were as follows: multiple regions (n = 4339, 49%); head and neck (n = 2351, 27%); and intrathoracic (n = 902, 10%).
These variables were age of the patient at the time of diagnosis, number of involved lymph nodes, tumor size, presence of multiple tumor, location of the tumor, presence of perigland growth, receptors for estrogen and progesterone, S-phase fraction, DNA index, DNA ploidy and if the patient has received PMRT treatment.
The largest subgroup of patients (61%) had one involved lymph node, 26% had two to three involved lymph nodes, and 11% had four or more involved lymph nodes.
Moreover, on multiple Cox regression models, two parameters of axillary involvement were indicative of outcome: the presence of more than three involved lymph nodes and extracapsular spread.
In terms of their primary staging, 20 patients had tumours measuring <20 mm with no involved lymph nodes detected (T1N0), 4 patients had 1 3 involved lymph nodes detected, 2 patients had tumours measuring >20 mm with no involved lymph nodes detected and 38 high-risk patients had >3 involved lymph nodes at surgery.
In esophageal cancer, selective removal of involved lymph nodes could improve survival and limit complications from extended lymphadenectomy.
Conversely, in B-CLL involved lymph nodes, a positive LM-332 reticular mesh was diffusely evident, throughout the disrupted nodal architecture.
One case study of a patient with sarcoidosis showed a mild 18F-FLT uptake and intense 18F-FDG uptake in the involved lymph nodes [30].
In 8/19 (42.1 %) patients, SRT was extended to pelvic and/or retroperitoneal LNMs including dose escalation for involved lymph nodes (Fig. 1).
