While the composition of the primary care team does not guarantee the delivery of best-practice care, which in addition requires a sound clinical quality-assurance system, appropriate funding, and well-trained clinicians, the achievement of good chronic disease outcomes for patients will remain elusive in the absence of a primary care team with the appropriate mix of competencies.
