In addition, supporting documents, i.e., case closing reports, miscellaneous case notes and emails, tuberculin test reports, NVSL laboratory reports, and an affected herd spreadsheet, were available for each bovine TB case either in paper or electronic format.
Information from each confirmed bovine TB case were obtained by review of the case closing reports, miscellaneous case notes, the tuberculin test record reports, NVSL laboratory reports, the affected herd spreadsheet and evaluation of each epidemiological investigation.
Miscellaneous cases included diseases such as Behçet disease.
The miscellaneous cases were 58 cases, including suspected cases which did not fulfil the criteria of clinical CAM (54 cases) or unknown cause (4 cases).
The lung cancer cases included 46 adenocarcinoma, 42 squamous cell cancer, 12 large cell cancer, 3 carcinoid, 3 small cell cancer, 2 BAC and 2 miscellaneous cases.

The index diagnosis was rheumatoid arthritis in 27 cases, idiopathic or posttraumatic osteoarthritis in 90 cases, and "miscellaneous" in 18 cases.
The major reasons for nonparticipation were refusals (73 cases, 7.4%), death before interview (36 cases, 3.7%), inability to locate the subject (22 cases, 2.2%), absence during the study period (10 cases, 1.0%), and other miscellaneous reasons (9 cases, 0.9%).
