Transiliac biopsies and corresponding lumbar spine BMD measurements from 31 women and 16 men with vertebral fracture were compared with those from 22 women and 11 men without fracture.
Prostate specific antigen should also be measured in men with vertebral fractures and symptoms of prostatism or evidence of sclerosis on x-rays.
A case-control study from the Mayo Clinic investigated 105 men with vertebral fractures and 105 age-matched control subjects with Paget's disease of bone.
In addition to improving bone density in men with hypogonadal osteoporosis (Behre et al 1999), testosterone may increase spine bone density in eugonadal men with vertebral fractures and there have been a number of small studies.
In an uncontrolled study in 42 men with vertebral fractures, cyclical etidronate increased spine BMD by 3% annually, whilst hip bone density showed a non-significant rise of 0.7% per year (Anderson et al 1997).
An uncontrolled study of testosterone treatment in 21 eugonadal men with vertebral osteoporosis showed a significant increase in spine bone density of 5% in six months, but no change in hip bone density was seen (Anderson et al 1997).

When pulmonary dysfunction was characterized according to restrictive and obstructive ventilatory impairment, age-adjusted analyses showed that women and men with prevalent vertebral fracture had higher risk of severe obstructive ventilatory impairment than those with no vertebral fracture, odds ratio (OR) 5.18 in women (P = 0.029) and 2.72 in men (P = 0.003) (Table 6).
In our study, we found significantly lower FEV1/FVC% predicted in women and men with lumbar vertebral fracture than in those without fracture.
As the goal of treatment is to prevent clinical (symptomatic) fractures, our results in the context of the available medical literature indicate that treatment should be offered to men who meet WHO criteria for osteoporosis (that is, femoral neck bone mineral density of 0.558 g/cm or below), men with existing vertebral fractures, and men with a recent history of hip fracture.
It has therefore been suggested that treatment for osteoporosis should be considered in men with low trauma vertebral or hip fractures and evidence of osteoporosis or osteopenia at the lumbar spine or femoral neck, whereas the possibility of unrecognized antecedent trauma should be explored in those with normal bone density measurements (Tuck and Francis 2006).
