Preoperative memory functional magnetic resonance imaging was the strongest predictor of verbal and visual memory decline following anterior temporal lobe resection.
Our findings demonstrate that both hippocampal volume and memory performance are correlated with functional activation within the to-be-resected hippocampus, hence explaining why these are risk factors for memory decline following ATLR.
A novel finding of our study was that while ipsilateral anterior hippocampal activation was associated with greater verbal and visual memory decline following left and right ATLR, respectively, relatively greater activation in the posterior part of the ipsilateral hippocampus (which is likely to be spared during an ATLR) was correlated with better verbal or visual memory outcome.
In patients with refractory temporal lobe epilepsy, evaluation of preoperative verbal and visual memory function is important as anterior temporal lobe resections may result in material specific memory impairment, typically verbal memory decline following left and visual memory decline after right anterior temporal lobe resection.
After left ATLR patients typically show verbal memory decline (Hermann et al., 1995; Sabsevitz et al., 2001; Gleissner et al., 2002, 2004; Bonelli et al., 2010) and less consistently, non-verbal memory decline following right-sided resections (Gleissner et al., 1998; Lee et al., 2002).
Individuals with greater left than right activation in this region were at greater risk of suffering a clinically significant verbal memory decline after left ATLR, while those with greater right than left activation were found to be at risk of suffering a clinically significant visual memory decline following right ATLR.
