A 19-year-old female with no significant past medical history or medication use presented to our hospital in April 2010 with severe nephrotic syndrome characterized by marked fluid retention, heavy proteinuria (protein creatinine ratio of 1244 mg/mmol), hypoalbuminaemia (13 g/L) and elevated total serum cholesterol (9.3 mmol/L).
The items 'There is an actual list of medication used present in the practice for every patient' and 'The practice has a control programme for oral anticoagulants when prescribed and dosed by the practice' were scored highest in this group and fifteenth overall.
Patients are enrolled in the cohort as they start treatment on the monitored medicine (treatment initiation); demographic information and medical information on the patient's disease status, pregnancy status, past medical history, medication use and presenting symptoms is captured at this initial encounter.

14 In contrast, the self-reported questionnaire item for medication use analysed in the present study asks participants a single question for each condition ('Are you currently taking the following medications?'), and high validity was observed, indicating that this mode of questioning is appropriate and reliable.
