In the event of hypoventilation or apnoea, respiratory effort was promoted by cutaneous stimulation, repeated doxapram boluses (1 mg/kg, maximum three doses) and, if necessary, reinstitution of nasal IPPV (rate 2 to 30 bpm).
Induction was done by intracervical dinoprostone gel (maximum three doses) followed by oxytocin infusion.
