Time management, self-management with an explicit focus on time in deciding what to do; on how much time to allocate to activities; on how activities can be done more efficiently; and on when the time is right for particular activities.
These indicators include other domains of care effectiveness, efficiency and safety: for example; symptom management, self-management and efficiencies for community providers.
Nurse-led care coordination consists of elements of disease and case management, self-management and caregiver support, which are derived from several chronic care models [ 26- 29] and adapted for the Dutch healthcare system.
