A paper could be classified into multiple linkage categories.
The broad linkage categories were "direct collaborative activities", "agreed guidelines", "communication systems" and "service agreements".
We then recoded the 42 randomised trials according to these broader linkage categories for comparison of linkages against outcomes.
Each paper was assigned to at least one of these four linkage categories, with some papers counted in two categories.
