Occupational physical activity was based on reported main lifetime occupations, coded according to the Australian Standard Classification of Occupations [ 26] and independently assigned by two of the authors (PHL, AR) as follows: sedentary, standing or manual (including heavy manual work).
As we journey through a lifetime, our birth code vibrates out a message to everyone we interact with.

For patients who underwent lumbar spinal fusion surgery (5.4% of the population with the inclusion of ICD-9-CM code 721.3), lifetime costs for nonoperative care were estimated at $142,994 per patient compared to $87,503 for MIS; the cost differential of $55,491 per patient would result in an estimated savings to Medicare of up to $1.4 billion over patients' lifetimes.
Self-reported alcohol consumption (average drinks/week), current and ever-smoker status (at least 100 cigarettes during lifetime), and medication use (coded according to the World Health Organization's Anatomical, Therapeutical, and Chemical Classification/Defined Daily Dose [ATC/DDD] codes) were obtained.
