We undertook a search for peer reviewed, English language studies using Medline, Embase and Cinahl between 1st January 2000 and 21st February 2014 following consultation with a university librarian with database and search strategy expertise (Additional file 2).

A systematic search of literature was performed with assistance of a clinical librarian in the medical databases National Guideline Clearinghouse, Trip database, Bandolier, British Medical Journal Clinical Evidence, Medline (January 1966 through May 2007), Embase (January 1980 through May 2007), and Cochrane Library, comprising the Database of Systematic Reviews (1988 through May 2007).
