Using the same process, the following TKR utilization process items were also selected for inclusion in the mediation models: having a discussion about surgery with a doctor, having been referred to an arthritis specialist, and not having been referred to a surgeon (Additional file 4).> -wrap-foot> aAdjusted for recruitment site, sex, age, income and Western Ontario McMaster Index total score.
The rest of the items were considered useful for local improvement processes and discussion and included to provide additional information regarding the work environment in the primary care setting.
