A superficial irregular ulcerative area was observed in the middle to lower third of the thoracic esophagus (Figure 1a), with an elevated lesion covered by normal epithelium found caudal to the main lesion (Figure 1b).
A hyperechoic, irregular band over the superficial margin of the articular cartilage - described as a double contour sign or icing - is found exclusively in gouty arthritis [ 8] and represents crystalline precipitates of MSU.
The most useful elemental lesion is the double-contour sign – a hyperechoic, irregular band over the superficial margin of the joint cartilage, produced by deposition of MSU crystals on the surface of the hyaline cartilage, which increases the interface of the cartilage surface, reaching a thickness similar to the subchondral bone.
Thick irregular enhancement of both deep and/or superficial fascia can also occur.
These may be, for example, a pronounced and irregular feeding vessel, the presence of superficial necrosis, ulceration, and so forth.
The scanning electron microscopy examination of a single spot showed grain loss and a superficial crater with a irregular fiber base encircled by smooth and intact grain (Fig. 4 and 5).
Other areas are affected macroscopically but at irregular depth and distribution, with the more superficial layers and ulnar side of the palm being affected most.
The light microscopic examination showed superficial grain loss and a irregular fibre base in the affected areas as compared with the intact grain in the surrounding tissues (Fig. 3).
The general pattern of the superficial layer could be cobblestone or irregular honeycomb pattern.
Microscopy examination showed extensive superficial mycotic proliferation, with wide and irregular ribbonlike nonseptate hyphae and right-angle branching.
