The second step involved standardising principles for attributes in KPI, that is, care dimensions (figure 2A, see online supplementary appendices B and C) and the study care domains to be tested, in this case six (figure 2B).
The second balancing loop (B2) represents the limitation caused by the extra costs of conducting a study involving standardised data.
This could involve standardising parts across manufacturers.
CD interviews involve incorporating standardised follow-up questions to understand how the intended population interprets the instructions, questions and response options, to enable improved clarity and appropriateness of wording (identifying any ambiguity in items and/or response options), to identify repetition and redundancy, and to identify additional items that need to be included [ 23].

Given the complexities involved in measuring and standardising oesophageal pressure measurements, and the preliminary nature of these findings, we believe that confirmation in the form of a larger multicentre RCT is required before this approach is used in the majority of ARDS patients.
However, this is likely to be minimal as most information in medical records of the hospitals involved in the study is standardised for the care practices considered.
This work has involved the development of rating schemes to standardise and report outcomes.
