By using the average of study specific estimates for β0, β1 and β2 with docetaxel as the reference, the expected β0, β1 and β2 for the other interventions were calculated using the relative treatment effects d0 Ak, d1 Ak, and d2 Ak. (See Table 4) The corresponding hazard and survival functions for each of the four interventions are presented in Figure 5 and 6A.
Lastly, provision of the service by skilled volunteer facilitators has ensured an acceptable, low-cost intervention with a rate of effectiveness in these four pilot groups that compares favourably to national non-targeted interventions outcomes calculated using standard assessment formula.
At $196 per child, the net cost of High Five for Kids falls within the range of four U.S. school- and community-based childhood obesity interventions, all calculated using a micro-costing approach and converted to 2011 dollars [ 20].
The cost of the goal-setting intervention was calculated using the number of staff hours spent developing and delivering the intervention.
