The efficacy of demonstration projects: A widely used strategy for promoting education reform is to demonstrate the effectiveness of an intervention on a small scale in a way that is highly visible to a wider audience, thereby creating a local demand for wider implementation of the intervention.
Experience of delivery of the intervention on a smaller scale enabled PCP to meet the majority of requirements for successful scale-up as described by Simmons et al. (2006)[ 24] and summarised by Gilson and Schneider (2010)[ 19].
Theories include a gravitational slingshot resulting from a near-collision with another planet, or the intervention of a smaller planet-like body working to gradually change WASP-17b's orbit by tilting it via the Kozai mechanism.
Through the case study, it was possible to evaluate the process and relationships when using this type of training intervention for a small group of radiology resident trainees.
It follows that opportunities for initial "trialling" of interventions on a smaller scale, the co-option of reflective front-line practitioners with tacit, insider knowledge on how to manoeuvre in the system and the presence of strong collaborative networks – both formal and informal – greatly enhance implementation.
Moreover, trial staff delivered screening and brief intervention to a small proportion of patients (5%) owing to flagging recruitment.
Unfortunately, the effects of these interventions are small, especially in the long-term [ 20– 20].
There is a lack of guidance on how to select an appropriate theory for a particular purpose (Michie, 2008), with a predominance in published intervention evaluations of a small number of theories that have already gained recognition in the field (Painter et al., 2008).
The intervention consisted of a small group educational session that was delivered by a trained Filipino health professional at community organizations, distribution of free Fecal Occult Blood Test (FOBT) kits and print materials, a reminder letter to participants, and a letter to their providers encouraging them to recommend CRC screening to this and all their eligible patients.
This rather large-scale recruitment intervention is followed by many interventions of a smaller scale: Continued Medical Education sessions for GPs for 10 to 50 participants during 1 to 1,5 hours and GP office visits for mostly 1 to 10 GPs during 30 to 60 minutes.
