George Ortiz, who was the Education Department's school bus route and safety inspection supervisor for all of Manhattan until 2007, was given 30 months in prison.
For some reason, a track inspection supervisor, who is required to reinspect the tracks once every 10 days, did not reinspect that stretch for several months before the derailment.
In addition, according to Transit Authority documents, corroded rails must now be marked with bright orange paint, and track inspection supervisors must reinspect all of those locations at least weekly to ensure that cracks are not forming.
The report added that 53percentt of the signal equipment on the A, B, C, D, E, F and G lines throughout the city and the R line in Brooklyn was overdue for backup visual inspections by supervisors at the end of 1999.
In the future, this information system can be used as an electronic assistant supervisors conducting inspection on any instructions or manuals.
They have asked for an extra month to prepare for the change at seven smaller stations: hiring 70 more technicians and supervisors, expanding inspection hours and taking appointments to prevent long waits for drivers.
The investigation, triggered last fall by an anonymous letter to the inspector general, whose office conducts independent investigations within the M.T.A., is focusing on whether the signal maintenance supervisors falsified inspection reports to say that the back-up inspections had been carried out, when in fact they had not been, the person said.
John Kincaid, supervisor of inspections for the department, said the Santa Monica Market is "a good clean market offering fresh, quality produce," and has incurred only a few violations over the years.
"It's a first step toward getting us to where we need to be". Debra Goode, a registered nurse with the county's health facilities inspection program, told the supervisors that the new contract will help to "position us for future success". "We know how critical the work is that we do in protecting the frail, the infirm and the developmentally disabled in Los Angeles County," she said.
"The allegation was that because the supervisors' inspections were behind, all of a sudden management decided they had to catch up, so hundreds of extra inspections were entered into the database as having been done that had not been done," the person said.
Site based data collection entailed interviews with accident-involved personnel and their supervisor or manager, inspection of the accident location, and review of appropriate documentation.
