Using the template analysis approach [ 18], a flexible coding structure was initially developed based on the major components of the Expanded CCM (self-management/develop personal skills, delivery system design/re-orient health services, decision support, information systems, build healthy public policy, create supportive environments; and strengthen community action) [ 9].
