The City of Cleveland has asked the family of Tamir Rice, the 12-year-old who was fatally shot while holding a toy gun last November, to hold off its civil suit until the criminal investigation into the incident is complete.
The officer has voluntarily suspended himself until an investigation into the incident is complete, according to Bill Stoeffler, a spokesperson for the police union the Commonwealth Constables Association.
The San Mateo Sheriff's Department said it will release the materials once an investigation into the incident is complete, although the department did not respond to a request for further comment.
When a device incident had occurred, patients and their families always were notified, manufacturers were contacted, and hospital incident forms were completed.
"Even in cases where it appears the officer has followed all guidelines, a final determination is not made until the full investigation into the incident has been completed," said John J. McCarthy, the department's chief spokesman.
Over the study period, 16 critical incident forms were completed by members of faculty from a cohort of 228 students.
Only 17.3% of charts with an AE contained documentation that indicated an incident report was completed, while 4.8% of charts without an AE had such documentation.
This meant that some of the early incident reports were completed on paper-based versions of the same reporting template and sent via the internal post to the risk management team who manually entered these data into the new system.
In addition, the fact that 4.8% of cases without an AE contained evidence that an incident report was completed suggests that what is reported does not necessarily correspond to the definition of an AE that was used in this study and that HC programmes may have different criteria for generating incident reports.
He was unable to say how many reports on these incidents had been completed but he also defended the Saudi intervention in Yemen.
