There is consensus among clinicians and auditors that this improvement can be attributed to the national reforms launched during 1995 2000 and their continuing momentum throughout the 2000s, manifest in two revisions to the national strategy and growing emphasis on outcomes monitoring (Department of Health, 2007, 2011).
