In practice, Level 2a and Level 2b were not clearly distinguishable because when an attempt to pull the reward failed in Level 2a training, the following trial had a Level 2b setting.

Figure 1 shows the magnitude and direction of differences in practice-level mean scores on 13 patient experience questions after case-mix adjustment is applied.
Clear boundaries of the network of agents and relationships involved in practice-level change would be hard to define, since communicative interactions both within and outside the practice would influence the pattern of organizing.
Case-mix adjustment accounted for 1 12% of the variance in practice-level mean scores (see column 1), and changed the relative ranks of between 2% and 11% of practices in pairwise comparisons (see column 2).
The two questions on access to care which related directly to payments to general practitioners (PE7 and PE8) had intra-class correlations of 0.079 and 0.125 respectively, resulting in practice-level reliability coefficients of 93.2% and 95% at the overall mean number of 262 responses per practice (with a mean number of 160 and 134 responses for the two payment questions).
As can be seen in figure 1 these differences in practice-level means are generally 5 10 points on the 0 100 scale (corresponding to 20 50 percentile points of change in rank, see appendix, figure A1), with the larger outlier adjustments almost entirely positive, that is, representing practices whose mean scores increased as a result of case-mix adjustment.
