Data were cleaned to check that the comments assigned in the positive or improvement boxes did actually belong to those categories and, if they did not, they were correctly re-assigned.
In recognising this definitional limitation, the UK Health Foundation has identified a framework that comprises four principles of person-centred care which should underpin any improvement intervention (box 1).
In line with the aims of the study, the 10 'challenges' from the published guidance for large-scale improvement initiatives (Box 1) were used deductively as a framework and as a programme-level theory to structure the analytical process.
They described being listened to, given time (which helped foster their sense of selves), thoroughly examined (which gave a sense of hope that something was being done) and provided with access to other support through referrals to professionals and services (which was perceived as both practical help and symbolic of improvement) (see Box 4).

Interviews with patients explored: (1) experiences of being certified and registered; (2) the impact of certification and registration on the lives of patients and their families and (3) suggestions for improvement (see boxes 2– 4).
