These included offering a range and balance of services, providing quality training for staff, exercising effective leadership and management and having effective strategies for identifying families in the community, to name just several of 18 distinct dimensions of implementation subject to quantitative scoring.
Needs are to be assessed through the use of evidence-based guidelines and protocols which are regularly updated, and their implementation subject to local audit.
The primary outcome of 1 year mortality was observed in 39/79 (49%) pre-implementation subjects and 77/206 (37%) post-implementation subjects (difference 12%; P = 0.04).
Between November 2004 and December 2005, 161 pre-implementation subjects were recruited.
Compared to pre-implementation, post-implementation subjects had a significantly lower ED SBP (72 vs. 85 mm Hg, P < 0.001) and higher sequential organ failure assessment score (7 vs. 5, P = 0.0004).

Another possibility for our findings could be related to a Hawthorne effect, caused by heightened awareness of the clinical staff that resulted in a different response to post-implementation subject's clinical needs.
In both cases, after finishing the implementation, the subjects had to pause the chronometer and ran the test units to verify whether their code was correct or not.
