Adding to the complexity of HF management is multimorbidity, 10 which makes the care of patients, and implementation of effective interventions and programmes more challenging.
Furthermore, as identified both in the WHO guidelines 8 and a recent WHO-led priority setting exercise, 45 there is a growing recognition of the need for research on improving implementation and scale-up of effective interventions and programmes for adolescents.
Our findings suggest that the implementation of intervention programmes similar to ours could have a major impact on patient safety by reducing the incidence of CRBSIs, and that routine replacement of PVCs might additionally prevent a significant number of bloodstream infections.
Telephone-based interventions are also being used successfully for other chronic disorders [ 32- 34], and while personal contact with health professionals is still a preferred method, there is change towards communication via other routes, for example the internet [ 32, 35], which further supports the tele-based implementation of intervention programmes.
