Monitoring community members' attitudes toward study activities and promoting community involvement were key elements of intervention development and helped study staff to troubleshoot implementation problems and implement adaptations with the support of intervention communities.
We focus on barriers in three stages of the policy process: 1) determining effective policies, which occur between policy formulation and decision making; 2) coordinating policymakers around the policies, in advance of implementation; and 3) implementing identified policies, as labeled in Fig. 1.

An analytical framework comprising six stages of change, divided into three phases, was used: pre-implementation (create awareness, commit to implementation); implementation (prepare to implement, implement) and institutionalisation (integrate into routine practice, sustain new practices).
However, the complexity of describing and implementing such optimized implementations is immense.
An open source reference implementation was designed and implemented to convert forms between ODM and CDA format.
It is envisioned that through this study, the chronic obstructive pulmonary disease-Integrated Care Pathway team will be able to identify potential gaps in the programme implementation and design, and implement necessary changes to improve care.
