The qualitative findings are supported by quotations from participants followed by a reference, for example (FG1, I, providers).
O'Connor et al. [ 2] postulated three classes of factors leading to CI: factors related to (i) providers, (ii) patients, and (iii) the system, with an estimated relative contribution of 50%, 30%and20%0% respectively [ 2].
(IA2, I, providers & experts) A number of the professionals considered provider incentives to be 'insulting' and 'unethical' as 'having a [professional] relationship with a woman' and associated increases in smoking quit and breastfeeding rates were the only incentive required.
The three provider-related factors that were initially defined by Phillips et al. [ 1] are assumed to be the most common contributors to CI [ 2, 4, 9]: (i) providers' overestimation of the care they give; (ii) providers' use of 'soft' reasons to avoid therapy; (iii) providers' lack of education, training or organisation for achieving therapeutic goals.
These were: i) providers' role in the health centre, their typical routine, and their position in relation to others in the facility; ii) challenges faced in day-to-day work; iii) perceptions of the work patterns and work culture in the facility, including relationships with colleagues and health centre managers; and iv) understanding of, and attitudes towards, the introduction of HIV services.
(IA2, I, providers & experts) At its best, it's a very positive, and very, re-enforcing of the good that you are doing, which makes people feel good about themselves on all levels, from the health care assistant right up to the manager, if it is handled properly, they feel very good about themselves, and they are incentivized to go forward because of that feeling.
