He had onset of illness on September 21 with headache, myalgia, and fever, and on September 24 he was admitted to a hospital in Lusaka.
He experienced onset of seizures at 6 months with a self-limiting GTCS.
Nine to 12 hours after the incident, he had onset of gastrointestinal symptoms (nausea, abdominal cramps, and loose stools [approximately eight nonbloody loose stools over the next 8 hours]), nasal congestion, postnasal drip, and a self-reported fever.
Asked about his health — only because he suffered a stroke in 2004 — he volunteered more information than he had to, allowing that he had early onset Parkinson's disease.
On arrival at LRMC (day 6 of illness), the patient had a poor prognosis for survival: ribavirin treatment had been delayed >4 d after symptom onset; he was somnolent; and he had severe bleeding and coagulopathy, significantly elevated levels of hepatic transaminases, a platelet count of <20,000/mm, and a serum viral load of >1 × 10 copies/mL (4, 23 – 23 ).
At day 9 postillness onset, he remained lucid and interactive, but he continued to have episodic high fever (temperature >39°C) and progressive dyspnea developed, which resulted in a need for supplemental oxygen.
Fifteen years ago, when he returned to the reservation from Tufts Medical School, he saw the onset of diabetes in patients in their 50's and 60's.
He missed the onset of the last recession, in 1990, and he was no more prescient about the slowdown that began at the end of last year.
